What happens next?

 

1. By our next working day, we will review the information.

2. From here, we will send a text message to your phone to put a card on file. If you inquired about sleep support, we will also be sending you a more in-depth intake form.

3. Once all intake forms are complete AND the card is on file,  we will connect you with your provider directly to schedule.

Please remember, after your appointment is completed, the remaining charge for the services you received will occur.
