Liability Waver
Lingua Speech, Swallow, and Voice Services LLC Workshops, Classes, and Events

By participating in any class, workshop, or event offered by Lingua Speech, Swallow, and Voice Services LLC (“Lingua”), you acknowledge and agree to the following terms:
1. Assumption of Risk
I understand that activities offered by Lingua—including but not limited to movement sessions, craft workshops, educational seminars, and wellness events—may involve physical, cognitive, or creative engagement that carries inherent risks. These may include minor injuries, allergic reactions, emotional discomfort, or aggravation of pre-existing conditions. I voluntarily choose to participate and assume full responsibility for any risks, injuries, or damages that may occur.
2. Medical & Personal Considerations
I affirm that I am (or the participant is) physically and mentally able to participate in the selected activities. I have consulted with a healthcare provider if I have any concerns. I understand that Lingua does not provide medical advice, diagnosis, or supervision, and that participation is at my own discretion.
3. Release of Liability
I hereby release, waive, and discharge Lingua Speech, Swallow, and Voice Services LLC, its owners, instructors, facilitators, affiliates, and agents from any and all liability, claims, demands, or causes of action arising out of or related to any loss, injury, or damage that may be sustained while participating in Lingua’s events.
4. Responsibility & Conduct
I agree to participate respectfully, follow all safety guidelines, and inform the facilitator of any limitations or concerns prior to the event. I understand that I am responsible for my own safety, well-being, and personal belongings during all sessions.
5. Photography & Media Release (Optional)
I grant permission for Lingua to use photographs or video taken during events for promotional purposes. I understand I may opt out by notifying the facilitator in writing prior to the event.
6. Acknowledgment & Agreement
By checking the box below or signing electronically, I acknowledge that I have read, understood, and voluntarily agree to the terms of this waiver and release.
